For your first visit, please bring your insurance card and a photo ID and arrive 15 min early to complete the necessary forms.We accept MEDICAID as well as most commercial insurers, however, please review all insurance information with our staff prior to services being rendered.
Your health insurance contract is between you and your insurance company. Any complaints regarding your coverage should be directed to your carrier.
Self Pay Policy – No Medical Insurance no problem. We accept patients without insurance. Uninsured patients will be required to make payment at time of service.
